Akciğer Cerrahisinde Lazer Kullanımı

Abstract

Modern tıpta, toraks cerrahisinde minimal invaziv tekniklerin önceliğinin artması, lazer kullanımına olan ilgiyi de arttırmıştır. Kansız cerrahide etkin rol oynayan lazerler bazı fiziki ilkelere sahiptirler. Tüm lazerlerin ortak özellikleri, monokromatik, koherent ve kolime edilebilir olmalarıdır. Lazerlerin toraks cerrahisinde, özellikle de akciğer cerrahisinde kullanımı iyi bilinmektedir. Yayılan ışık dalga boyları ve dokuları pıhtılaştırma, kesme veya buharlaştırma güçleri ve yetenekleri bakımından akciğer parenkiminde kullanılan farklı birçok lazer türü vardır. Akciğer parenkiminde uygulanılabilenlerde, hücre içi su absorbsiyonu, kesme, hemostaz ve koagulasyon özelliği yüksek, parenkim pnömostazının istenen lokal etkilerini üreten ideal lazerler aranır. Bunlar arasında; Diyod, Nd:YAG ve Thulium lazer en çok kullanılanlardandır. Özellikle akciğer dokusunda metastaz olgularında minimum doku kaybına neden olması gerekçesiyle lazerler tercih edilirler. Bu derleme ile lazer fiziği ve mekanizması üzerinde durularak akciğer dokusunda lazer kullanımı hakkında bilgi verilmesi amaçlanmıştır.

Application of Laser in Lung Surgery

Abstract

The increasing priority of minimally invasive techniques in thoracic surgery in modern medicine has also increased the interest in laser application. Lasers, which play an active role in bloodless surgery, have some physical principles. The common features of all lasers are that they are monochromatic, coherent and collimable. The application of lasers in thoracic surgery, especially lung surgery, is well known. There are many different types of lasers used in lung parenchyma in terms of the wavelengths of light emitted and their power and ability to coagulate, cut or vaporize tissues. Ideal lasers with high intracellular water absorption, cutting, hemostasis and coagulation properties and producing the desired local effects of parenchymal pneumostasis are sought for those that can be applied in the lung parenchyma. Among them; Diode, Nd:YAG and Thulium laser are the most used ones. Especially in lung tissue metastases, lasers are preferred because of causing minimal tissue loss. In this review, it is aimed to give information about the use of laser in lung tissue by emphasizing the physics and mechanism of laser.
