Radioulnar Angular Deformiteli Köpeklerde Korrektif Osteotomi ve Osteoplasti Sonuçlarının Değerlendirilmesi

Abstract

Amaç: Bu çalışmanın amacı, köpeklerde radioulnar angular deformitelerin CORA metodolojisine göre angulasyon derecesinin belirlenmesi ve korrektif osteotomi ve osteoplasti sonuçlarının değerlendirilmesidir. Gereç-Yöntem: Çalışmanın materyalini, ön ekstremitelerinde topallık ve carpal eklemlerinde açılanma şikayeti ile getirilen 14 köpek (n=14) oluşturdu. Tüm olgulara preoperatif ve postoperatif topallık, ortopedik ve radyolojik muayeneler yapıldı. Topallık skorlaması yapıldı. Radyografilerde dirsek ve carpal eklemler için oryantasyon açıları, medial proksimal radial açılar (MPRA) ve lateral distal radial açıyı (LDRA) veren anatomik eksen ve eklem oryantasyon çizgilerinden açılar ölçüldü. MPRA ve LDRA arasındaki fark alındı ve elde edilen bu açı frontal düzlem açısı olarak belirlendi (FPA). Etkilenen ekstremitelerin valgus deformitelerinin operatif planlaması CORA yöntemine göre yapıldı. Genel anestezi altında, cerrahi olarak 4 olguda ulnektomi, 10 olguda korrektif osteoplasti gerçekleştirildi. Bulgular: Tüm olguların postoperatif ortopedik muayenelerinde carpal eklemlerde efüzyon, ağrı ve krepitasyon algılanırken; topallık muayenesinde, genellikle 1 ve 2. derece topallık belirlendi. Topallık 3 olguda yoktu. Postoperatif FPA ortalama değeri 7,5±4,07° olarak ölçüldü. Cerrahi sonrası tüm olgularda FPA’nın normal değere yaklaştığı saptandı. Sonuç: Köpeklerde radioulnar angular deformitelerin düzeltilmesinde uygulanılan korrektif osteotomi ve osteoplasti teknikleri ile her ne kadar normal anatomik situsa yakın bulgular sergilenmese de cerrahi olarak başvurulan bu yöntemlerin klinik olarak anlamlı sonuçlar gösterdiği vurgulanabilir.

Evaluation of Corrective Osteotomy and Osteoplasty Outcomes in Dogs with Radioulnar Angular Deformity

Abstract

Objective: The aim of this study was to determine the degree of angulation of radioulnar angular deformities in dogs according to the CORA methodology and to evaluate the results of corrective osteotomy and osteoplasty. Material and Methods: The material of the study consisted of 14 dogs (n=14) brought with the complaint of lameness in their anterior extremities and angulation in their carpal joints. Preoperative and postoperative lameness, orthopedic and radiological examinations were performed in all cases. Lameness scoring was done. Orientation angles for the elbow and carpal joints, medial proximal radial angles (MPRA) and lateral distal radial angle (LDRA) were measured on the radiographs. The difference between MPRA and LDRA was taken and this angle was determined as the frontal plane angle (FPA). The operative planning of the valgus deformities of the affected extremities was performed according to the CORA method. Under general anesthesia, ulnectomy was performed in 4 cases and corrective osteoplasty was performed in 10 cases. Results: In the postoperative orthopedic examination of all cases, effusion, pain and crepitation were detected in the carpal joints. In the lameness examination, generally 1st and 2nd degree lameness was determined. Lameness was absent in 3 cases. The mean postoperative FPA value was 7.5±4.07°. It was found that FPA approached the normal value in all cases after surgery. Conclusion: Although corrective osteotomy and osteoplasty techniques applied in the correction of radioulnar angular deformities in dogs do not show findings close to normal anatomical situs, it can be emphasized that these surgical methods show clinically significant results.
